This Liability Waiver ("Waiver") is executed by "Participant", or if the Participant is a minor, by their parent or legal guardian ("Guardian"), on behalf of the Participant.

1. Acknowledgment of Risk The Participant acknowledges and agrees that participation in dance activities, classes, rehearsals, or events held at K Dance Studio involves inherent risks, including but not limited to physical injuries, strains, sprains, or other unforeseen accidents. The Participant voluntarily assumes all risks associated with such activities.

2. Release and Waiver of Liability In consideration of being permitted to participate in activities at K Dance Studio, the Participant (or Guardian) releases, waives, and discharges K Dance Studio, its owners, directors, employees, contractors, agents, and representatives (collectively referred to as the "Releasees") from any and all claims, liabilities, or demands of any kind, whether caused by negligence or otherwise, arising from participation in any studio-related activity.

3. Medical Consent In the event of an injury or medical emergency, the Participant (or Guardian) authorizes K Dance Studio to seek medical treatment deemed necessary and agrees to bear all costs associated with such treatment. The Participant (or Guardian) shall provide any pertinent medical information, including allergies or pre-existing conditions, to the studio staff before participating in activities.

4. Covenant Not to Sue The Participant (or Guardian) agrees not to initiate any legal action against the Releasees for injuries, damages, or losses sustained during activities at K Dance Studio.

5. Indemnification The Participant (or Guardian) agrees to indemnify and hold harmless the Releasees from any claims, liabilities, or damages arising from their participation, including those brought by third parties.

6. Rules and Policies The Participant agrees to abide by all rules, regulations, and policies set by K Dance Studio, including but not limited to:

· Proper use of studio equipment and facilities.

· Adherence to safety protocols.

· Respectful behavior towards instructors and other participants.

7. Photo and Video Release The Participant (or Guardian) grants permission for K Dance Studio to use photos or videos taken during activities for promotional purposes, including on social media, the studio’s website, or other marketing materials.

8. Governing Law This Waiver shall be governed by and construed in accordance with the laws of the State of Georgia.

9. Entire Agreement This Waiver constitutes the entire agreement between the parties regarding liability and supersedes all prior agreements. Any modifications must be in writing and signed by both parties.
